About the role

The physiatrist will round at multiple skilled nursing facilities to deliver specialized care and develop individualized treatment plans for patients with pain or functional limitations. Responsibilities include performing targeted interventions like joint injections and Botox while maintaining autonomy in a collaborative clinical environment.

Day-to-Day Responsibilities As a physiatrist, you will round at 1–3 Skilled Nursing Facilities or similar rehabilitation centers, 1–3 days per week at each location.

Your primary focus will be delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.

Key responsibilities include: Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.

Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.

Serving as the sole physiatrist on-site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.

Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.

This role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.

Qualifications

Current, unrestricted PA or NP license. DEA licensed or willing to obtain DEA licensure. Nursing home and/or physiatry experience is a plus, but not required. Comfort and interest in performing peripheral joint injections (knees, shoulders, hips). A genuine passion for providing professional, personalized care in an autonomous environment. Strong communication skills, with the ability to collaborate with multidisciplinary teams. Familiarity with EHR charting Why Iconic Care?
