Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care. Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care. Practice accepted infection control principles. Must hold a current CNA license and have at least one year of experience in a hospice, nursing home, or home health setting. A high school diploma or GED is preferred, along with a valid driver's license and auto insurance.
