Summary Provides professional nursing care through skillful assessment, diagnosis, outcomes identification, planning, implementation, and evaluation in accordance with facility or department policies procedures. Directs and manages the care of designated patients. Utilizes effective and appropriate communication styles. Assists in maintaining a safe work environment and maintains clinical and professional competency as appropriate to the population of patients served.

Job Duties

Performs a comprehensive subjective and objective assessment using standardized assessment tools to gain pertinent details about the patient's overall health, well-being, abilities, and living environment; performs on-going assessment of physical, psychosocial, and environmental parameters.

Delivers patient care based on the medical plan of treatment established by the physician and protocols using a patient family centered approach. Reviews, evaluates, and as appropriate, revises the established plan of care for each patient.

Collaborates with the interdisciplinary team and other members of the patient's care team in assessing and supporting psychosocial needs of the patient/family members/caregiver.

Provides educational opportunities for patients, families, and clinical staff focusing on end-of-life issues, palliative care, advance directives, chronic disease management, pain management, symptom control, home care, hospice, and discharge planning.

Promotes shared governance, facility decision making activities, and developing and nurturing research to positively affect clinical outcomes and promotion.

Utilizes appropriate resources to plan, provide, and sustain evidence-based nursing services that are safe, effective, and fiscally responsible.

Directs and guides ancillary personnel and maintains standards of professional nursing.

Minimum Qualifications

  • Specialized Diploma Nursing
  • 1 year Recent direct care experience in a healthcare setting.
  • Ability to build comprehensive reports and effectively convey information to others.
  • Ability to deal with crisis situations and make independent judgments.
  • Knowledge of patient education techniques and principles.
  • Knowledge of home care Conditions of Participation.
  • Skill in providing patient care, ensuring safety, and quality of nursing care.
  • Basic computer skills and ability to perform duties with accuracy and attention to detail.
  • Demonstrates flexibility and adaptability in varying situations to meet patient care needs.
  • Demonstrates initiative and ability to function independently without direct supervision and as a member of the care team.
  • American Heart Association Basic Life Support - State of Pennsylvania within 30 Days
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire
  • DL - Driver's License_PA - State of Pennsylvania Upon Hire

Preferred Qualifications

• Bachelor’s Degree Nursing

Physical Demands Lift and carry 40-50 lbs.

Examples

Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
