Provide direct patient care through office visits and telehealth, including comprehensive assessments and personalized treatment plans. Collaborate with a multidisciplinary team to educate patients on preventive health and functional medicine principles.
Requires an active Oregon license as a Nurse Practitioner, PA-C, MD, or DO with board certification in a relevant specialty. A minimum of 2 years of clinical experience is preferred, along with proficiency in electronic health records.
