Provide occupational therapy services to adult and geriatric patients within skilled nursing facilities. Establish individualized plans of care and maintain accurate clinical documentation including progress notes and discharge summaries.
Candidates must hold a valid State Occupational Therapy Assistant License and relevant credentials. Previous experience in skilled nursing facilities or acute care settings is required.
