The Case Manager provides behavioral health rehabilitation services and support to veterans and their families to improve community functioning and reduce crisis episodes. Responsibilities include managing a caseload, conducting counseling sessions, maintaining accurate clinical documentation, and coordinating community resources.
Candidates must hold a Bachelor’s degree in a behavioral health-related field and qualify as a Mental Health Professional (MHP) under Illinois state rules. Experience in behavioral health services and a valid driver's license are required, with preference given to those with military experience.

Provides behavioral health rehabilitation services and support to veterans and families to decrease hospitalization and crisis episodes and to increase community functioning for the client to achieve rehabilitative, resiliency, and recovery goals. Provide continuity of program services in an ethical, legal, and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Participate as a team member in the delivery of behavioral health services to clients and their families and facilitate the issues of recovery into their daily living situations.
