The RN provides coordination of care and continuous assessment of hospice patients' physical, medical, and emotional needs. They work within an interdisciplinary team to implement and manage the patient's plan of care.
The candidate must be a Registered Nurse (RN) capable of working in a home care setting. They are expected to demonstrate professionalism, collaboration, and a commitment to the hospice mission.
