As a member of an interdisciplinary rehabilitation team, this position is responsible for nutritional evaluation, treatment planning and implementation, documentation, and patient/family education for outpatient cardiac and pulmonary clients, as well as referrals received for members of the community.

The incumbent also serves as a resource for the cardiopulmonary department team members.

The incumbent also performs other professional duties and participates in department/facility initiatives as assigned.

Responsibilities

A day in the life of a Dietician at Hackensack Meridian Health includes: Prepares for the evaluation in accordance with department policies/procedures and standards of practice, including obtaining patient history, exhibiting knowledge of patient diagnoses and researching unfamiliar diagnoses. Selects and administers tests and measures. Additionally, synthesizes evaluation results to formulate the diagnosis, goals, and plan of care. Establishes a treatment plan based on evaluation results, goals and best practice. Collaborates with an interdisciplinary team, patients, and patients' support system. Assesses and re-assesses need for modifications to plan of care and goals. Identifies need for consultations and/or refers the patient to other health care professionals or health care services. Initiates and completes discharge plan. Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies and third party payers which reflects an understanding of reimbursement systems and their impact on patient care. This includes, but is not limited to, examination, evaluation, progress notes, reassessments/progress reports, discharge plans and communication with members of the team. Additionally records daily billing and follows department guidelines for productivity. Education - Provides education/training to patient and/or patient's support system. Maintains updated educational materials. Participates in community education events as requested and schedule permits. Schedules patients according to department guidelines, which includes both in person and virtual appointments. Other duties and/or projects as assigned. Adheres to HMH Organizational competencies and standards of behavior.

Licenses and Certifications Required: Registration with CDR for Registered Dietitian certification within 6 months of hire date. Licensure or Certification as required by local state regulations. AHA Basic Health Care Life Support HCP Certification - BLS. Licenses and Certifications Preferred: Diabetes Education. Requirements

Candidates must hold a bachelor's degree in a nutrition-related field and possess or be eligible for Registered Dietitian certification. Basic computer literacy and BLS certification are required, with clinical experience preferred.
