Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join CapitalCare Family Practice Clifton Park, a well-established primary care practice providing comprehensive care to patients of all ages and committed to coordinated, high-quality, patient-focused care.

Position overview

As an Advanced Practitioner at CapitalCare Family Practice Clifton Park, you will deliver full-spectrum family medicine services in a collaborative, team-based environment. You will practice to the full extent of your license with strong physician support, integrated clinical services, and scheduling designed to promote work–life balance and long-term professional satisfaction.

Responsibilities

  • Conduct comprehensive patient evaluations and manage acute and chronic conditions
  • Order and interpret diagnostic studies, prescribe medications, and coordinate referrals as needed
  • Provide preventive care, health maintenance, and patient education
  • Collaborate closely with physicians, nurses, and support staff to ensure continuity of care
  • Maintain accurate, timely clinical documentation in accordance with organizational standards

  • Care across the lifespan – serving pediatric, adult, and geriatric populations
  • Structured five or four-day schedule available (8:00 AM – 4:30 PM) with a consistent daily workflow and protected midday administrative/lunch time

  • Established, team-based environment with multiple physicians and Advanced Practitioners supporting a strong patient panel
  • Convenient Clifton Park location, serving a growing suburban community in the Capital Region

Compensation: Starting at $125,000 for full-time

CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.

Qualifications

  • License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
  • Physician Assistant:
  • Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program.
  • Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
  • Current certification by National Commission on Certification of Physician Assistants.
  • Current CPR certification.
  • Nurse Practitioner:
  • Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
  • Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
  • Successful completion of pharmacology coursework acceptable to NYSED.
  • Three years of professional nursing experience in an ambulatory setting preferred.
  • At least one year of experience as a nurse practitioner in ambulatory health care.
  • Current CPR certification.

Requirements

Candidates must possess an active New York State license as a Physician Assistant or Nurse Practitioner, along with specific educational and certification requirements for their respective roles. A minimum of one year of relevant ambulatory healthcare experience is required for PAs, while NPs prefer three years of nursing experience and at least one year as an NP.
