The RN Care Coordinator facilitates efficient patient transitions through the acute care process by collaborating with a multidisciplinary team. They ensure quality-driven, cost-effective care while maintaining compliance with CMS guidelines and quality improvement standards.
Candidates must hold a current Registered Nurse license in Virginia and possess at least three years of acute care nursing experience. A Baccalaureate degree in Nursing or a related health/social science field is required.
