About the role

The provider will work autonomously and collaboratively with multi-disciplinary teams to deliver palliative care services across multiple hospital locations. Responsibilities include symptom management, end-of-life care, and assisting with patient consults and new admissions.

We are seeking an Advanced Practice Provider to join one of the nation’s most integrated healthcare systems, with over 1,300 providers in Indianapolis, Indiana.

Highlights Community Physician Network seeks a Nurse Practitioner or Physician Assistant to join our Palliative Care team in Indianapolis, IN.

The most comprehensive palliative care program of its kind in Indiana, our team is committed to helping patients and families understand where they are on their medical journey and providing the physical and emotional comfort they need.

0.6 FTE position, weekdays This provider will work autonomously and collaboratively with our palliative care multi-disciplinary teams as a "float" provider at our network hospitals (Community Hospitals North, East, South, Community Heart & Vascular Hospital, Community Howard Regional Hospital, and Community Hospital Anderson) to deliver palliative and health care services to patients.

Focus on the comfort measures of patients, end of life care, symptom management, as well as assisting with consults and new admissions.

Flexibility to travel between hospitals in Indianapolis Competitive compensation and benefits package, including incentive potential

Qualifications

Ideal candidates will: Possess 1-3 years’ Experience as a licensed Nurse Practitioner in Palliative Care Be comfortable assessing patients independently Be compassionate caregivers with excellent organizational, communication, and critical thinking skills Possess highly developed interpersonal and group dynamic skills, as well as proven skills as a consultant/educator to friends, families, and loved ones Licensure & Certifications Educational background consistent with requirements for licensure and certification within the state of Indiana About Us Headquartered in Indianapolis, Community Health Network has been deeply committed to the communities it serves since opening its first hospital, Community Hospital East, in 1956.
