The Contract Care Coordinator Assessor will conduct initial assessments for new enrollees to identify health needs and develop individualized care plans. They are responsible for connecting members with necessary resources and maintaining accurate documentation of care transitions.

Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Join our team as a Contract Care Coordinator Assessor where you will work from home, but also be required to travel about 30% of the time to meet with members in Hennepin County, MN.
Looking for part-time OR full-time position contractors (please submit work availability with online application). Flexible timeframe of work within regular business hours.
Approximately 70% work from home & 30% in-person travel to meet with members in Hennepin County, MN (Mainly throughout Minneapolis and St. Paul, MN with mileage is reimbursed per company and IRS guidelines).
Paid per assessment - Averages $35 per hour (hourly amount within range is based on experience).
ASAP thru September/October 2026 with the potential of Temp to Full-time opportunities available.
