Conduct clinical assessments, provide behavioral therapy, and manage medications for patients in skilled nursing facilities. Coordinate care with interdisciplinary teams and maintain accurate patient notes in electronic health records.
Requires an advanced practice nursing degree and an unrestricted license in the state of practice. Possession of a DEA certificate and the ability to meet credentialing requirements for Medicare and Medicaid are mandatory.
