What You’ll Do

The Clinical Dietitian Diabetes Educator will serve as the go-to resource for diabetes education while also providing comprehensive dietitian services across our hospital and clinics. In this role, you’ll make a direct impact on patients’ health journeys—from acute inpatient care to long-term outpatient management. You’ll have the opportunity to build meaningful relationships with patients across a variety of settings and be a trusted educator and advocate in their long-term health.

Serve as the primary diabetes educator, providing individualized diabetes self-management education and skills training to patients of all ages, from pediatric to geriatric, in alignment with ADCES guidelines.

Partner with patients and families to support lifestyle changes, prevention of complications, and long-term health outcomes through one-on-one and group education.

Provide nutrition services and medical nutrition therapy for hospitalized patients and outpatients, ensuring care aligns with patients’ medical goals.

Perform nutrition assessments, create care plans, monitor progress, and provide education and counseling in multiple settings.

Write orders for diets, supplements, and enteral/parenteral nutrition per facility policy, and monitor related laboratory data.

Collaborate closely with physicians, nurses, and other clinical staff as an integral part of the care team.

Travel between hospital and clinic locations to meet patient and program needs.

Support performance improvement initiatives and ensure compliance with state, federal, and accrediting agencies.

Occasionally work flexible hours, including on-call, evenings, or weekends, as needed.

Who We’re Looking For

Maintains Registered Dietitian (RD) status including continuing professional education per the Professional Development Portfolio of the Commission on Dietetic Registration.

Certified Diabetes Care and Education Specialist (CDCES) status with the National Certification Board of Diabetes Educators required within 1 year of beginning the role or mentorship under accredited clinic. Maintains CDCES with throughout GRHD employment.

Must have current BLS card within 3 months of initial employment and maintain current status throughout GRHD employment.

Prefer two years’ or more professional experience in a clinical or outpatient setting in a similar role.

Compensation may vary outside this range depending on a number of factors, including a candidate’s qualifications, skills, competencies and experience.

Requirements

Candidates must hold Registered Dietitian (RD) status and obtain Certified Diabetes Care and Education Specialist (CDCES) certification within one year. A minimum of two years of professional clinical or outpatient experience is preferred.
