The Palliative Care Social Worker provides psychosocial assessments and evidence-based interventions for patients and families facing serious illness. They serve as a member of the interdisciplinary team to alleviate stress and provide specialized bereavement support.
Candidates must hold a Master's degree from an accredited School of Social Work. At least two years of experience in palliative, end-of-life, or medical social work is preferred, along with relevant Pennsylvania licensure.
The Palliative Care Social Worker provides psychosocial assessment and evidenced based psycho-social intervention for palliative care patients and their families. A member of the Palliative Care interdisciplinary team, Palliative Care Social Workers utilize their skills and knowledge of person-centered system based clinical skills to alleviate stress associated with serious illness and end of life care. Palliative Care Social Workers provide specialty bereavement and grief interventions to patients, families, and staff across the healthcare system.
