The Case Manager provides support and case management for individuals with mental illness and co-occurring disorders to assist them in living independently. They are responsible for driving clients to appointments and groups while working within a multidisciplinary team to promote recovery.
Candidates must have a high school diploma or G.E.D. equivalent. A valid driver's license and personal vehicle insurance are also required for the role.

Under direct supervision of the Team Leader, the Case Manager I provides support and case management for members served with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Expected starting wage range is $18.63 - $20.20. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
