The Advanced Practice Provider will provide primary care services to geriatric and disabled patients, including physical examinations, health assessments, and management of chronic conditions. They will also collaborate with medical specialties and coordinate community resources to meet patient needs.
Candidates must have a valid license and certification as a Nurse Practitioner or Physician Assistant, along with graduation from an accredited program. Prior primary care experience and comfort with technology are preferred.

Phix Health is looking for an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join us at an outpatient practice in and around Philadelphia, PA. This is a part time opportunity for 2-4 days per week, depending on location.
Provide compassionate primary care services for a geriatric and/or disabled patients including comprehensive and focused physical examinations, health assessments, diagnoses, and treatment of acute and chronic illnesses Manage chronic conditions such as diabetes, hypertension, congestive heart failure, arthritis, chronic obstructive pulmonary disease, and thyroid disorders Assess need for appropriate lab tests, order and interpret results Collaborate with medical specialties through appropriate referrals Assist in coordination of community resources to meet needs of individuals and families, as well as appropriate follow-up. Educate patients on health promotion throughout the aging process Evaluates and treats patients in accordance with Phix Health’s Care Model. Establishes and monitors appropriate level of care for center patients. Completes all medical record documentation prior to end of shift. Works with Medical Director to ensure care is operations are consistent with medical and professional standards of care. Recommends potential growth opportunities for new or existing services. Meets with Medical Director or designate to discuss quality of patient care, review policies, procedures, outcomes reports, and records. Participates in quality management processes including encouragement of clinical support staff to assure proficiency in the delivery of clinical support activities. Calls patients/family member to ensure their well-being as needed following center visits. In the absence of Medical Director, provides guidance to other medical personnel and center colleagues. Perform all other related duties as assigned.
Assures personal compliance with licensing, certification and accrediting bodies.
