Provide psychosocial support and assessments for hospice patients and their families in both community and facility settings. Facilitate discharge planning, coordinate with interdisciplinary teams, and assist families with healthcare goals and end-of-life arrangements.
Requires a Bachelor's or Master's degree in Social Work and 1-3 years of healthcare experience. A valid North Carolina Clinical Social Worker license (LCSW) is preferred, along with a valid driver's license.

Provide social work services and support that ensures the patient and their family/caregiver receive psychosocial services according to the standards of Four Seasons Compassion for Life mission statement, policies, and procedures. SW works in a variety of locations including the IPU and the community based on need.
• Bachelor’s in Social Work (BSW) OR Master’s in Social Work (MSW)
• 1-3 years experience within a healthcare setting
