The Nurse Case Manager assesses, plans, and implements health processes for assigned patient panels using the Dene' Model of Care. They coordinate healthcare services, advocate for patient needs, and collaborate with multidisciplinary teams to ensure quality care.
Candidates must hold an Associate degree in nursing and a valid RN license, with a Bachelor's degree preferred. At least three years of clinical experience and patient population health management experience are required.
