The Nurse Case Manager assesses, plans, and implements care processes for assigned patient panels using the Dene' Model of Care. They collaborate with healthcare teams to advocate for patient needs and ensure quality care coordination.
Candidates must hold an Associate degree in nursing and an active RN license, with a Bachelor's degree preferred. At least three years of clinical experience and patient population health management experience are required.
