The Case Manager provides professional social work services, conducts resident assessments, and facilitates interdisciplinary care coordination. They also supervise the Residency Coordinator and assist with discharge planning and family education.
A Bachelor's degree in social work is required, with a Master's degree preferred. Candidates should have 3-5 years of professional experience in a long-term care or assisted living setting.
Our Case Manager is a key member of our interdisciplinary care team, which is focused on providing person-centered care that is highly individualized and attuned to the specific needs and preferences of those we serve. Day-to-day responsibilities involve resident assessments and interventions, interface with other interdisciplinary team members, supervision of the Residency Coordinator and education of residents, family members, and staff. A love for working with Seniors is a must, as is having a passion for bringing healing and hope to those we serve. Successful incumbents will possess a nurturing disposition, strong interpersonal skills, and the compassion it takes to get the job done while building strong bonds and relationships.
