$45,000 - $55,000 / YEAR
Manage a caseload of 50 individuals with developmental disabilities by providing advocacy, education, and service linkage. Complete all required DHS mandated forms, visiting reports, and person-centered plans within specified time frames.
Requires a bachelor's degree in human services or a related field and at least one year of experience working with individuals with developmental disabilities. Candidates must be eligible for QIDP status and possess a valid driver's license with reliable transportation.

In order to meet caseload visiting needs, qualified candidates must reside within a reasonable commuting distance from one our office locations: Rockford, Crystal Lake, Aurora, Lombard, Joliet, and Kankakee.

· Bachelor’s Degree in Human Services or a related field (Required) · Minimum one-year of personal or professional experience working with individuals with developmental / intellectual disabilities (Required) · Eligible for QIDP status (Required) Service Inc provides QIDP certification training. · QIDP certified from the Department of Human Services including completion of the DHS approved 40-hour training course (Preferred) · Valid Driver’s License, Insurance, and reliable transportation (Required) · Bilingual encouraged to apply · Ability to meet Essential Job Functions with or without reasonable accommodations (Required)
