Provide personalized occupational therapy to patients in their homes to help them regain independence and perform daily activities. Collaborate with a multidisciplinary home health team to assess patient needs, establish therapy goals, and educate families on care plans.
Must be a graduate of an accredited Occupational Therapy program with an active Colorado license and at least one year of home health experience. Candidates must also possess CPR certification and pass a background check and drug test.
