The Community Health Worker will coordinate care by assessing patient needs, providing health education, and connecting families with clinical, social, and financial resources. They are also responsible for maintaining patient documentation, tracking outcomes, and advocating for individual and community health.
Candidates must have a high school diploma or equivalent. A strong understanding of community-based resources and experience as a direct care worker are required.
