  • Hospice Savannah Inc. - SAVANNAH, GA 31406Summary The PACE Occupational Therapist is responsible for evaluating, assessing, and implementing therapeutic interventions designed to encourage and allow participants to engage in meaningful daily activities and enhance functional independence through a holistic approach utilizing acute and maintenance occupational therapy interventions. The PACE Occupational Therapist is a member of the PACE Interdisciplinary Team (IDT) and is responsible for completing occupational therapy assessments for new PACE participants and occupational therapy reassessments on existing PACE participants as part of the PACE IDT care planning process." QualificationsQualifications Master’s degree in Occupational Therapy, Preferred Doctorate degree in Occupational Therapy. Minimum of one to two years of work experience as an Occupational Therapist One year of experience working with a frail or elderly population, or in the absence of such knowledge, receive appropriate training on working with a frail or elderly population upon hiring (CMS requirement). Must have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act. (CMS requirement). Have not been excluded from participating in Medicare and Medicaid programs. Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse (CMS requirement). Physical Demands and Work Environment This position requires frequent physical activities such as standing, walking, and performing fine motor tasks like pinching. Occasional duties include sitting, climbing stairs or ladders, carrying objects, pushing or pulling, twisting, bending, reaching forward or overhead, squatting, kneeling, crawling, deviating wrist position, and repetitive keyboard use. Continuous communication is essential, with ongoing requirements to talk and hear. The role involves lifting up to 75 lbs from floor to waist and up to 50 lbs from waist level and above, along with sensory demands for accurate near vision, color discrimination, minimal depth perception, and precise hearing. Employees may encounter occupational exposure risks including bloodborne pathogens, chemicals, airborne communicable diseases, uneven surfaces or elevations, and dust or particulate matter.
