Provide individual and group rehabilitation therapy services to adults with serious mental illness within a multi-disciplinary team. Responsibilities include planning educational outings, documenting client progress, and implementing Recovery plans.

Telecare will deliver and manage excellent services and systems of care for persons with serious mental illness.
With the support of the Rehabilitation Therapist Supervisor, the Rehabilitation Counselor provides rehabilitation therapy services to clients, both on an individual and group basis, and serves as a member of the multi-disciplinary Recovery Team.
Shifts Available: On-Call | Shifts and days vary as needed
Expected starting wage is $22.55.
This position is covered by a collective bargaining agreement and is subject to union membership requirements upon hire.
Telecare applies geographic differentials to its pay ranges.
The pay range assigned to this role will be based on the geographic location from which the role is performed.
Starting pay is commensurate with relevant experience above the minimum requirements.
Quality and quantity of work
Provides safe, effective and efficient implementation of direct care in accordance with established policies, procedures and standards of care.
Establishes and maintains a therapeutic relationship with Clients.
Plans and conducts large and small activity groups under the indirect supervision of the Director of Rehabilitation. This includes: 1. Maintaining awareness of Clients’ physical and psychiatric limitations and monitoring the safety of Clients at all times. 2. Enlisting the cooperation of nursing staff in supporting the structured activity program. 3. Facilitating Clients’ attendance at scheduled activities and focusing efforts on maximizing Client involvement. 4. Accurately documenting each Client’s attendance and response to activity and rehabilitation groups in the medical record.
Plans and conducts outings for Clients which are educational and entertaining. When assigned, drives the facility van on outings.
Develops a theme and schedules a series of special events for assigned months.
Judgment, decision making and initiative

Promotes and assists Clients in developing self-help skills.

Continually focuses on assigned tasks and seeks and implements improvements as necessary.

Demonstrates a knowledge of Telecare’s customers including clients/Clients, families and governmental agencies in all interactions and conduct.
