Key Responsibilities Assess patients’ healthcare needs, develop individualized care plans, and coordinate services across multiple providers.
Serve as a primary point of contact for patients and families, providing education, support, and advocacy.
Collaborate with physicians, nursing staff, social workers, and community resources to ensure comprehensive patient care.
Monitor patient progress, evaluate outcomes, and revise care plans as necessary.

Valid and active Registered Nurse (RN) license.
