The Case Manager coordinates with other departments to provide maximum services and demonstrates the necessary knowledge and skills to provide care based on various criteria. This includes physical, psychosocial, educational, and safety-related factors appropriate to the age of the patients served.
Candidates must have a minimum of two years of clinical experience in a hospital setting, preferably with a background in Utilization Review or Discharge Planning. A current professional Texas RN licensure is required, along with the ability to work with multidisciplinary staff and knowledge of Medicaid and Medicare guidelines.
Graduate from an accredited School of Registered Nursing.
