House Calls Nurse Practitioner - Full Time - Company Car
Location: Virginia (Field-based; patient homes within service area)
Employment Type: W-2 Full-Time
Reports To: Medical Director (Clinical) | Practice Administrator (Operations)
What You’ll Do: · Provide in-home primary care for chronic disease management, dementia care and preventive health needs
· Develop and manage individualized care plans tailored to patient goals
· Perform Annual Wellness Visits (AWVs), follow-ups, Chronic Care Management (CCM) and Transitional Care Management (TCM)
· Coordinate care with specialists, home health, hospice, DME providers and community agencies
What You Bring: · Active Virginia Nurse Practitioner license
· National certification (AANP or ANCC)
· DEA registration or eligibility
· Medicare and Virginia Medicaid enrollment (preferred)
· Experience in primary care, geriatrics or home-based care (preferred)
