The Physical Therapist will deliver personalized, high-quality outpatient therapy in home and community settings to help clients achieve their goals. They will also collaborate with a multidisciplinary team and educate clients and caregivers on safety and wellness strategies.
Candidates must hold a current New Jersey Physical Therapist License. A minimum of one year of outpatient or home health experience is preferred, along with strong communication and time management skills.

Are you a compassionate and driven Physical Therapist ready to make a difference? At Fellowship Therapy at Home, we’re redefining outpatient therapy by bridging the gap between home health and clinic-based services. We provide care in clients’ homes—delivering therapy where it matters most.
We are seeking a Full-Time Physical Therapist to join our growing team in Monmouth County. This is a full-time position ranging from 30–40 hours per week based on caseload.
