The Integrated Care Team Social Worker coordinates quality healthcare services to meet individual medical, rehabilitative, and financial needs. They provide emotional and mental health support while helping community members overcome social determinants of health barriers.
Candidates must hold a current Maine licensure in social work and a bachelor's degree in social work. A minimum of two years of experience in the field and a valid driver's license are required.

Education, Training or Certifications Required: 1. Current Maine licensure in social work. 2. Bachelors degree in social work from an accredited institution. 3. AHA Basic Life Support. 4. Valid Drivers License. Experience Required: 1. Minimum two years of experience in the field of license. 2. Demonstrated knowledge of health, social service, and funding sources. 3. Experience in community and hospital social work and discharge planning preferred. 4. Experience counseling individuals and groups with behavioral, emotional and behavioral disorders. 5. Motivational interviewing experience preferred. Special Requirements: 1. Within the scope of practice and license, must have the ability to work independently with limited supervision, capability to make independent decisions regarding counseling, education or referral of patients. Maintains confidentiality. 2. Reliable personal vehicle or mode of transportation. Cognitive Requirements: 1. Emotional maturity, resilience and stability. 2. Knowledge of best practice standards and ethics. 3. Ability to multitask, prioritize and organize. 4. Ability to form collaborative, proactive and patient-focused relationships to focus, facilitate and maximize client healthcare outcomes. 5. Demonstrated communication and leadership skills with healthcare team members to facilitate maximal client outcomes. Physical Demands: 1. Able to sit, stand, walk for 8 hour shifts. 2. Able to carry 20 pounds. 3. Effectively communicates verbally and written.
