The social worker will assess the psychosocial needs of hospice patients and their families while providing emotional support and crisis intervention. They will also coordinate community resources, assist with advance directives, and maintain accurate clinical documentation.
Candidates must hold a master's or bachelor's degree in social work from an accredited program and possess a valid driver's license. A minimum of one year of experience in health care, preferably in hospice, is required.
Provide various personal caregiving services in accordance with an established plan of direct care. Provide for the personal needs and comfort of people in their own homes Assist patients with activities of daily living including bathing, dressing, grooming and toileting Prepare meals and special diets Perform general housekeeping activities Provide transportation assistance to and from activities Provide compassionate companionship to patients Other duties as assigned
