About the role

The provider is responsible for assessing, diagnosing, and treating patients in a family practice setting while working under physician supervision. They will also manage treatment plans, order diagnostic tests, and coordinate patient referrals.

Affiliates of Family Medicine is looking for a full-time Mid-Level Provider—either a Nurse Practitioner or Physician Assistant—to join our healthcare team in our Conroe office. In this role, you will provide primary care services by evaluating, diagnosing, and treating patients while working under the supervision of a collaborating physician.

Position Summary

The Nurse Practitioner/Physician Assistant is responsible for assessing, diagnosing, treating, and educating patients in a family practice setting. Working under physician supervision, the provider applies advanced clinical knowledge and skills to develop and implement treatment plans while monitoring and evaluating patient outcomes.

Essential Functions

  • Evaluate patients by reviewing medical histories and performing appropriate physical examinations, including comprehensive (360) exams
  • Interpret patient data and collaborate with the supervising physician to determine appropriate treatment and management plans
  • Prescribe and manage medications as appropriate
  • Maintain accurate and timely documentation, including evaluations, treatment plans, progress notes, and other required reports
  • Order and interpret diagnostic tests to identify abnormal conditions
  • Coordinate referrals to specialists, healthcare facilities, and community resources as needed
  • Ensure a safe, clean, and compliant work environment by adhering to all policies and regulations
  • Perform additional duties as assigned

Requirements

  • Current Nurse Practitioner or Physician Assistant (PA-C) license
  • Master’s degree from an accredited Nurse Practitioner or Physician Assistant program
