Salary
$20 - $23 / HOUR
Assist patients and community members in accessing and applying for various state and federal assistance programs. Conduct outreach to vulnerable populations and collaborate with health workers to connect clients to essential services.
Requires knowledge of community resources, strong data-entry skills, and the ability to communicate clearly. A Peer Support certification and a valid driver's license with auto insurance are preferred.
