The RN Case Manager will provide compassionate care to hospice patients while educating caregivers on how to support their loved ones. They will collaborate with the interdisciplinary care team to ensure patients receive dignified and high-quality end-of-life care.

Candidates must hold a current RN license in the state of Missouri and possess at least two years of registered nursing experience. A valid driver's license, CPR certification, and BCLS certification are also required.
