About the role

The Social Worker/Clinical Therapist manages the patient admission process, identifies resident needs, and provides ongoing counseling. They are also responsible for developing acute care plans, conducting safety assessments, and coordinating with community resources.

Primary responsibilities

  • Provides social work and discharge planning for patients who require acute mental healthcare.
  • Facilitates therapy groups and provides crisis intervention.
  • Collaborates with multidisciplinary team members to provide age/developmentally appropriate care in accordance with unit standards.
  • Complies with the objectives and policies of the hospital.
  • May be needed to work beyond the regular tour of duty or in another patient care unit as assigned by department head or nursing supervisor.

POSITION QUALIFICATIONS (including required licenses/certifications, education and job knowledge/experience):

LCSW degree/license required.

Recent experience in mental healthcare field

Current BLS Certification: Familiar with referral and state regulations for mental health patients

Effective oral and written communication skills in the English language

Ability to perform effectively in a fast paced, stressful environment

Customer service responsibilities

  • Performs duties and conducts interpersonal relationships in a manner designed to project a positive image of the department and the hospital.
  • Works with supervisor/manager and other members of section or department to promote a harmonious work environment.
  • Ensures that all contacts with patients, the public, physicians and other hospital personnel are carried out in a friendly, courteous, helpful and considerate manner.
  • Answers telephones in a prompt and courteous manner.
  • Displays concern and provides assistance or explains procedures as appropriate to callers or in face-to-face situations.
  • Dedicated to meeting the expectations and requirements of internal and external customers.
  • Works with customers to identify problem areas and recommend solutions.

Requirements

Candidates must hold a Master's degree in Social Work or Mental Health Counseling and possess an active LCSW or LMHC license. Previous experience in acute care or skilled nursing is required, along with BLS certification.
