Position Summary

Duke University Hospital is seeking an Advanced Practice Provider (APP) to join the Outpatient Abdominal Transplant Clinic. The APP will provide comprehensive, high-quality, evidence-based care to adult abdominal transplant patients across the pre‑ and post‑transplant continuum within Duke Health ambulatory practice settings. This role functions within a highly collaborative, interprofessional transplant care model and practices to the full extent of licensure and training in alignment with Duke Advanced Practice standards. The APP supports the Division of Abdominal Transplant Surgery by enhancing access, continuity, quality, and outcomes of care in a nationally recognized academic transplant program.

Clinical Responsibilities

Obtain comprehensive medical histories, perform focused and complete physical examinations, and synthesize clinical data to guide care Review, interpret, and act upon diagnostic studies, including laboratory results and imaging Develop accurate and comprehensive differential diagnoses and evidence‑based treatment plans Prescribe, manage, and adjust immunosuppressive and transplant‑related medications in accordance with departmental guidelines and credentialed privileges Provide longitudinal management of pre‑ and post‑transplant patients, revising plans of care as clinical conditions evolve Manage post‑transplant issues including rejection surveillance, infectious evaluations, and post‑operative complications Perform medical procedures within the scope of practice, credentialing, and departmental standards Maintain timely, thorough, and concise documentation to support care delivery, continuity, quality metrics, and compliant billing Prioritize patient flow and clinic efficiency while maintaining productivity expectations Participate in quality, safety, and clinical improvement initiatives specific to transplant care

Knowledge, Skills, and Abilities

Demonstrated clinical competence in the management of complex patients in a specialty outpatient setting Strong analytical, critical‑thinking, and clinical decision‑making skills Ability to practice both independently and collaboratively within a multidisciplinary transplant team Effective written and verbal communication skills with diverse patient populations and clinical stakeholders Strong interpersonal skills that support patient care, teamwork, and clinic operations Demonstrated proficiency in medical record documentation to substantiate clinical care and accurate billing

Education & Interprofessional Collaboration

Provide clear, compassionate education to patients and caregivers regarding transplant conditions, medications, monitoring, and follow‑up Collaborate with physicians, nurses, pharmacists, coordinators, and allied health professionals to ensure patient‑ and family‑centered care Assist in the development of educational resources to support learners and clinical staff Actively engage in continuing education and self‑assessment to support professional growth and clinical excellence

Interact with patients, families, and team members in a respectful, compassionate, and timely manner Communicate effectively across disciplines to ensure safe, coordinated, and high‑quality care Demonstrate accountability for clinical decisions, outcomes, and professional responsibilities Share responsibility for team workload and support a positive, collaborative work environment

Minimum Qualifications: Education

Nurse Practitioner

Licensure & Certification

Nurse Practitioner: Active or compact RN licensure in North Carolina National NP certification required

Physician Assistant: Current North Carolina PA licensure NCCPA certification

Must demonstrate and maintain credentialing and licensing requirements of the DHIP and Duke Health. The position requires the candidate to have current DEA licensure. Preference for candidates with prior advanced practice or RN experience with transplant surgery, general surgery, GI, Nephrology, Hepatology.

Candidates must hold a degree from an accredited Nurse Practitioner or Physician Assistant program and possess active North Carolina licensure. Preference is given to those with prior experience in transplant surgery, general surgery, GI, Nephrology, or Hepatology.
