What You'll Do In conjunction with the Medical Plan Of Care, independently perform systematic and continuous assessment of the physical, emotional, clinical, and psychosocial needs of assigned patients and families: plans and implements appropriate interventions; and evaluate effectiveness and adjusts plan as indicated in a timely manner in the provision of safe and effective care.
