The Clinical Partner assists physicians and nurse practitioners in delivering primary healthcare services within residential settings. Responsibilities include coordinating travel, preparing patients for examinations, performing clinical procedures like phlebotomy, and maintaining accurate patient records.
Candidates must have a high school diploma or GED and completion of an accredited medical assistant, phlebotomy, or EMT program. A valid driver's license and current CPR certification are required, along with basic computer literacy.
The Clinical Partner (Medical Assistant) will assist Harmony Cares Primary Care Physician, Nurse Practitioners and Clinical Teams in the delivery of primary health care and patient care management in a residential setting. Essential
Supports mission, vision and core values which include but not limited to delivering compassionate support, attention and assistance to patients, families and members of the team Support Patient Care through Clinical Partnership with Nurse Practitioners, Physicians, and Care Management Teams Coordinates daily travel itinerary and transports clinicians to scheduled appointments Prepares patients for examination and treatment; May at times include preparing patients for minor procedures Secures patient consent forms, records patient care documentation accurately and in a timely manner Coordinates patient care as directed by Clinicians, company standards and policies Respects patient confidentiality at all times and treats patients with courtesy and respect Performs phlebotomy (venipuncture only), EKG or any other procedure as requested by Clinician Labels, stores and properly packages specimens for lab delivery Assists In identifying and closure of quality gaps Documentation and updating of patient care plans, as well as SDoH (Social Determinants of Health) and other assessments within the patient chart or electronic medical record Required to maintain compliance with CLIA (Clinical Laboratory Improvement Amendments), OSHA, and HIPPAA guidelines Prepares and maintains supplies and equipment for treatment (supply inventory, medical bag and car) (where applicable) Ensure company vehicle is clean and proper periodic maintenance schedule is followed (where applicable) Attends required meetings and in-services In this role you may work with. . . Required Knowledge, Skills, and Experience High School graduate or GED equivalent Graduate of an accredited Medical Assistant, Phlebotomy, EMT, related medical education/training program or 1 year of experience serving as a Medical Assistant or Phlebotomist Current CPR certification that meets Company guidelines Knowledge of medical terminology Basic computer literacy, including EMR, iPad, and GPS experience Strong interpersonal communication and customer service skills Must maintain a valid driver’s license and good driving record Preferred Knowledge, Skills and Experience Certified Medical Assistant Six months of experience in patient care, preferably geriatric care Strong Phlebotomy skills Desire and ability to obtain MA certification with 180 days of hire Conditions of this role to be aware of. . . Full range body motion including handling/lifting patients Ability to drive up to 150 miles daily in urban and/or rural setting Ability to lift/carry items weighing up to 50 lb Ability to read and understand road maps Sit or stand for long periods of time
