The Private Duty Nurse provides skilled nursing services to clients in their homes while maintaining accurate clinical documentation and ensuring continuity of care. They are responsible for monitoring client conditions, managing emergencies, and building relationships with families to ensure compliance with care plans.
Candidates must be a graduate of an accredited nursing school and hold a current, unencumbered Licensed Practical Nurse (LPN) license in North Carolina. A minimum of one year of home care experience is required, along with a valid driver's license and reliable transportation.
Private Duty Nurse (PDN)-LPN The Private Duty Nurse (PDN)-LPN is a member of the client care team that provides services directly, or acts as a placement agency, to deliver skilled nursing services to clients in their personal residences. The PDN LPN works under the supervision of the Clinical Manager.

Wage/Hour Classification: Non-exempt
Evaluates and treats clients with appropriate level of skill according to client’s needs under the supervision of an RN, Clinical Manager. Assist with providing continuity of care and emotional needs of client and family. Resourceful in meeting client’s psychosocial needs. Verbal demonstration of client rights and responsibilities. Make decisions based on sound nursing judgements. Recognizes and copes with emergencies appropriately. Ensures proper management of client services. Demonstrates competent use of equipment. Appropriately communicates information relative to client care in accordance with agency policies. Observes the client’s general physical, emotional, and mental status, and reports all changes in the client’s conditions, family situation, incident, and occurrences during shift immediately to agency RN/CM or Customer Care Coordinator. Ensure all forms of communication with clients, families, and health plan providers are documented timely to promote continuity of care across all interdepartmental functions. Build relationships with clients to encourage compliance with care plans and client safety. Maintains accurate clinical and shift work documentations to update system in accordance with guidelines set forth by the agency in a comprehensive and timely manner. Manage client/contract satisfaction with the highest level of customer service standards, communication, and documentation. Adheres to infection control policies and exposure control plan as mandated. Monitor and maintain clinical report, including Infection and Occurrence/Complaint Reports. Adheres to and promotes safety and injury prevention measures in client and workplace settings. Maintain and improve skills through attendance with staff development programs and in-service education requirements. Participate in community outreach activities that promote the goals and objectives of the agency. Maintain Confidentiality in all professional areas and consider the needs of clients and their families, other employees, physicians, visitors, and all members of the community in an informative, courteous, and compassionate manner. Ensure all HIPAA regulations are adhered to. Follow Agency Policies and Procedures All other duties as assigned.
Routinely required to sit, stand, and walk on variable surfaces, including occasional climbing of ramps or stairs or the use of foot controls. Routinely required to use hands for various tasks, including hand controls, gripping, and handling of objects. At times, will be required to reach beyond mid-line of the body, reach overhead or at chest level. There are specific vision abilities required to perform this job, which include close, distance and peripheral vision, depth perception and ability to adjust focus. All employees will be required to use universal safety precautions when performing their assigned tasks. Employees must have the ability to lift, push, pull and carry a minimum of 25 pounds. Employees must have the ability to stoop and bend up to 2.5 hours per 8-hour shift.
Education: Graduate of an accredited school of nursing. Licensure / Certification: Licensed practical nurse (LPN) who is licensed by the North Carolina Board of Nursing (NCBON). Must have a current, unencumbered license as an LPN, or an Advanced Practice Nurse. Must be in good standing in the applicable state of practice. Must maintain all state and federal compliance requirements for licensure and employment.
