The Enhanced Care Coordinator provides case management, skills training, and coordination of services for individuals with mental illness. This role involves implementing treatment interventions, monitoring foster home providers, and collaborating with a multi-disciplinary team to reduce barriers to recovery.
Candidates must hold a bachelor's degree in psychology, social work, or a related field, along with at least two years of experience in a mental health setting. A valid driver's license and the ability to perform daily travel within the community are required.
Adapt Integrated Health Care is looking for a qualified Enhanced Care Coordinator to join their team! This position works Monday-Friday from 8:00 am to 5:00 pm
This classification coordinates and collaborates with a multi-disciplinary treatment team and is responsible for work performed in assigned specialized program area. The ENCC also will license and monitor Adult foster home providers for regulation with Oregon Administrative Rule providing direction and guidance.
