The Care Coordinator will assess children's functional abilities and develop individualized service plans to help them live safely in their communities. They will also facilitate team meetings, coordinate services with providers, and maintain accurate documentation in compliance with regulatory standards.
Candidates must hold a bachelor's degree in social work or a related human services field and have at least one year of experience working with children or youth with disabilities. A valid driver's license and reliable transportation are required for community-based travel.
