  • Range: $82,000.00 - $92,000.00 SalaryWe are seeking a highly motivated and clinically skilled Registered Dietitian (RD) to support nutrition services within a skilled nursing facility with a short-term rehabilitation program. This role is integral to interdisciplinary care, with a strong emphasis on active participation in care plan meetings and collaboration with nursing, therapy, and medical teams. The RD will ensure the delivery of high-quality, compliant, and resident-centered nutrition care that supports both long-term residents and short-term rehab patients. Key Responsibilities Conduct comprehensive nutrition assessments for long-term care residents and short-term rehab patients Develop, implement, and monitor individualized nutrition care plans Attend and actively participate in interdisciplinary care plan meetings (required) Provide ongoing evaluation of residents’ nutritional status, including weight trends, intake, labs, and skin integrity Collaborate with nursing, therapy, and physicians to support rehabilitation outcomes and recovery goals Ensure compliance with all CMS regulations, state requirements, and facility policies Provide guidance and oversight on therapeutic diets, texture modifications, and hydration programs Participate in survey readiness and support during state and federal inspections Educate residents, families, and staff on nutrition-related topics Partner with foodservice leadership to ensure menu compliance and resident satisfaction QualificationsQualifications Registered Dietitian (RD/RDN) credentialed with the Commission on Dietetic Registration Licensed Dietitian/Nutritionist in the state of [Insert State] (if applicable) Experience in skilled nursing, long-term care, or rehabilitation settings preferred Strong understanding of clinical nutrition, regulatory requirements, and care planning process Excellent communication and interdisciplinary collaboration skills Ability to manage multiple priorities in a fast-paced healthcare environment
