Provide skilled nursing care to clients in their homes under the direction of a Registered Nurse.
Conduct timely and thorough assessments and deliver treatments based on physician orders and care plans.
Educate clients and families on health conditions and self-care.
Document all care activities in accordance with agency protocols and EMR requirements.
Respond to client needs during assigned on-call shifts and per diem requests with professionalism and empathy.
Collaborate with interdisciplinary teams to ensure continuity and quality of care.
Licensed Practical Nurse (LPN) in the state of Connecticut 1+ year of nursing experience (home health preferred) Strong communication, critical thinking, and documentation skills Comfortable working independently in home settings Reliable transportation, valid driver’s license, and auto insurance Tech-savvy and familiar with EMR systems (training provided)
