The Case Manager provides comprehensive psychosocial assessments and develops individual service plans for consumers. They act as an advocate and liaison, coordinating care between providers, families, and community resources.
Candidates must hold a relevant license such as LBSW, LLBSW, LMSW, LLMSW, LLPC, LPC, TLLP, or LLP, or a bachelor's degree in a human service field with an SST certificate. Prior case management experience in a mental health or rehabilitation setting is preferred.
