The pharmacist will manage drug distribution systems, central operations, and IV admixture while providing clinical support in a hospital setting. They are also responsible for monitoring patient drug therapy and educating patients on medication use post-discharge.

Responsibilities

Receive admissions and/or transfers to the unit Initial and ongoing systematic patient assessment Timely and accurate documentation using appropriate systems Interpret assessment/diagnostic data including labs and telemetry Ensure medical orders are transcribed and processed accurately Competence in Rapid Response and code events Instruct and counsel patients/families Performs other duties as assigned

Qualifications

Minimum Qualifications

Valid State RN License BLS is required at hire. ACLS is required at hire.

Preferred Qualifications: Clinical experience Additional Data Equal Opportunity Employer/including Disabled/Veterans

Requirements

Candidates must hold a current state pharmacist license and have completed all educational requirements mandated for licensure. A PharmD or BS Pharm with an advanced degree or hospital pharmacy residency is preferred.
