The Case Manager provides intensive care coordination and monitoring for program participants while maintaining accurate documentation of services. They also facilitate interdisciplinary case conferences and assist in organizing health education classes.
Candidates must have a high school diploma or equivalent and at least five years of relevant case management experience. Proficiency in both English and Spanish is required, along with a valid driver's license and reliable transportation.
Minimum High school graduate or equivalent Five years' case management experience or equivalent combination of education and experience Bilingual English/Spanish Valid California driver's license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years' driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record Availability and willingness to work evening and weekend hours, as required Required Skills/Knowledge/Abilities Knowledge of co-occuring conditions Effective written/verbal communication skills Familiarity with local HIV/AIDS and non-HIV specific resources for health and social service needs Familiarity with health and social service needs of target populations served Basic computer knowledge, internet use, MS Office and data entry skills Pay Range $24.00 - $26.00
