CCH Social Worker

• As a CCH Social Work Case Manager SW/CM, you are responsible for supporting individuals with developmental disabilities who are living in DDS licensed private homes located throughout the State of Connecticut.
• In this role, the SW/CM is required to visit each home on their assigned caseload a minimum of one time per month.
• During each monthly home visit, the SW/CM will spend time with the individual(s) living in the home and their care provider.
• The SW/CM will be responsible to review the individuals personal file which contain medical, financial, and behavioral information and complete documentation following each visit within required timeframes.
• This role will work closely with a support team that includes a Nurse, Behaviorist, Supervisor, and DDS Case Manager to ensure that the individuals in your homes have continued access to the happy and healthy lifestyle that they deserve.
• In this role, the SW/CM will have the opportunity for a flexible schedule (Monday through Friday) in conjunction with the needs of the program.
• Office hours will be worked in our Main Office location of Newington.

The ideal candidate for this role is someone who is a self-motivated ‘people person,’ is flexible, and possesses strong organizational skills. Being a team player, as well as having strong writing skills are also preferred skills.
