• What You’ll Do: Conduct comprehensive psychosocial assessments and develop individualized care plans.
• Provide counseling, emotional support, and crisis intervention for patients and families.
• Connect patients and families with community resources and support services.
• Collaborate with an interdisciplinary team to ensure holistic, patient-centered care.
• Maintain timely, accurate documentation in compliance with regulatory and organizational standards.

Wage

• $28 - 32 per hour
