Provide high-quality primary and palliative care to homebound patients in the McDonough area by performing physical exams and developing treatment plans. Effectively communicate with clinical operations and manage a panel of patients while ensuring patient-centered care.
Must be a Board Certified Nurse Practitioner or Physician Assistant with a valid Georgia certification. Requires 1-2 years of primary care experience with geriatric patients and the ability to work independently.

We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients in the McDonough area of Georgia area on a full-time basis. You will apply medical techniques and principles under the supervision of a physician, and you will care for a diverse set of patients with chronic diseases.
• $105,000- $145,000+ (depending on experience and other incentives) (full Time)
